In brief: Strengthens surface

Fluoride helps prevent tooth decay by making the tooth more resistant to acid attacks from plaque bacteria and sugars in the mouth.
It also reverses early decay. A dentist can apply Fluoride to teeth in the form of a gel, foam, or varnish. These treatments contain a much higher level of Fluoride than the amount found in toothpastes and mouth rinses. However, best prevention is proper oral hygiene.

In brief: Caries prevention

Topical Fluoride is the best treatment for caries prevention and teeth sensitivity, since it replacing the loss of oh group by Fluoride ion during the enamel (hydroxyappatite) demineralization in the low ph environment.
It should be applied with and/or without the carrier (a tray fit over your teeth) as long as it remains on the tooth surface for at least 30 minutes.

In brief: Concentrate Fluoride

This gives added protection to the tooth structures.
Mostly used for enamel, but can also be used for root structures. You may or may not benefit from this treatment - ask your dentist to advise your particular situation.
