In brief: Is it symptomatic?
If is is symptomatic surgery is the best option.
If not, observation is your best bet.

In brief: Surgery
If symptomatic, the only treatment for syringomyelia is surgery.
Many patients these days are diagnosed with syringomyelia on an MRI scan peformed for some other reason or a symptom unrelated to the syringomyelia (e.g. Is found incidentally). In these cases, no treatment is necessary and simple observation is all that is required.
