Should I be concerned if my doctor still does routine episiotomies?

In brief: No
If you do not want an episiotomy then have that conversation with your doctor well before your labor starts and write it into your birth plan.
Ask questions about your doc's episiotomy rate and why he or she does them. Most docs do not routinely cut them anymore. If it is important to you then your doctor should respect your wishes and not cut one unless there is an urgent need.
