Very seldom is the size of the ear diminished during bilateral otopla sties (ear tucks).
Sometimes the ear lobe if too dependent (long) can be revised but not the cartilage structure. Often times the illusion of "too big ears" goes away when the ears are properly set back to the side of the head.

An otoplasty, or ear pinning, is the type of surgery done to treat ears that stick out too much.
It is possible to reduce the overall size of the ear at the same time, though this isn't necessary very often. If your ear lobes are contributing to this enlarged size, then they can be reduced at the same time as the setback of the ear. If your ear lobes are otherwise normal it can be more complicated to reduce the size of the ear. Your surgeon can give you a better idea about whether anything else can be done to improve this aspect of your ears.
