You should always have a breast lump evaluated by your doctor, but I agree it is usually caused by fat not getting enough blood, this will often get better with time.

The tissue that remains has a decreased blood supplie. If a portion of tissue does not get enough blood supply, it can die and turn hard.

Sometimes this is due to internal sutures, blood clots, or areas of fat that did not survive the surgery. Generally, these should resolve with time and some massage. Anything that persists should be investigated by your surgeon. Better to be safe.
