You should always have a breast lump evaluated by your doctor, but I agree it is usually caused by fat not getting enough blood, this will often get better with time.

This can sometimes be caused by fat necrosis (fat that died and turns hard) or just scar tissue.
Either way any masses or hard spots should be evaluated by your doctor and followed appropriately.

When reducing the breast a large amount of tissue is removed.
The tissue that remains has a decreased blood supplie. If a portion of tissue does not get enough blood supply, it can die and turn hard.

Sometimes this is due to internal sutures, blood clots, or areas of fat that did not survive the surgery. Generally, these should resolve with time and some massage. Anything that persists should be investigated by your surgeon. Better to be safe.
