I will add, as someone who does addiction medicine, that illicit use of methadone and other opiates can also decrease a man's ability to be a good father. If you or the person you're writing about is using opiates illicitly, I strongly encourage participation in a qualified methadone or suboxone treatment program and avoidance of other illicit drugs. I sincerely wish you & yours the best.

In brief: Possible

Chronic opioid therapy affects the natural production of sex hormones.
Frequently the testosterone level is significantly decreased, decreasing libido and sexual function. First choice is taper. Second choice is testosterone supplementation and viagra (sildenafil).

In brief: Yes

Methadone shuts down the hypothalamus which produces a hormone necessary for both testosterone and sperm production.
This can be corrected by a male fertility specialist.

In brief: It can

As its has been associated with hormonal abnormalities that can lead to low sperm counts.
Remember this doesn't happen to all men who take methadone though.
