Mastoiditis has increase risk because it is considered an infection of the mastoid bone which can lead to infection in the brain if severe or not treated.
A sinus infection is a infection within the sinus cavities which is not near the bone/brain barrier.

Both can lead to serious or even life threatening complications, but this is rare.
True or "coalescing" mastoiditis is very uncommon these days, with abx taking care of most serious ear infections. Sinusitis is much more common, abx take care of most of these infections as well. We see serious complications from both mastoiditis and sinusitis many times a year however.
