If dm is advanced (hypoglycemic unawareness) this is worth the risk. If you need a kidney transplant anyway, a pancreas transplant does not add much risk. The main risks of these operations is related to the presence of heart disease or hardening of the arteries, and need for long term is.

Correction-Immunosuppresants,not immuno modulators cause the bad side effects including kidney toxicity by cyclosporine with lowering of kidney function in later years. Slso the reversal of DM induced changes is not total. Some of the damage done already may not be reversible ,but overall improvement occurs with out further damage and reversal of major problems, it is defintely good if patient gets the transplant . Selected small population of DMtype 2 patientswho had the trasplantation also were found to have benefits significantly per a study done in UK. Reversal of nephropathy -kidney disease with and improved fertility in females are other proven benefits. Regarding the graft- in addn to failure of the graft,graft pancreatitis,infection necessitating removal of the transplant ,thrombosis of the grft can happen. Infections- secondary to being on immunosuppresants both due to some specific viruses and bactria are there.
