The mcl is a stabilizer of the medial knee and prevents the knee from opening up when stressed into a valgus (knock knee) position.
Its like a rubber band, many frequent small twisting motions won't tear the mcl, although it may may it sore. Twisting or pulling it beyond its normal physiologic limits and it will tear just like that rubber band.
