Keep your stools soft, avoid straining.
The formula for soft stools is to have plenty of fiber in your diet and keep well hydrated - easiest way to do that is to add soluble fiber (metamucil, citracel, etc.) 2 tablespoons once or twice daily, and drink plenty of fluids. Over the counter stool softeners (docussate, dss, or colace) can also help. Warm baths can also help soothe the area. Good luck.

From ur statement @ the end of ur post does it mean there was a tear on a rectal exam? If so it usually requires surgical intervention.
Blood on the tissue or on the outside of the stool can b due hemorrhoids internal or internal. These r aided by soft stool, tucs, etc. If concerned c a GI specialist or proctologist
