Vascular disease can be a cause of death and extensive disease implies poor control of the risks of atherosclerosis (smoking, in particular, but also diabetes, hypertension, hyperlipidemia, and familial risk (which you can't control of course).
Depending on which vessels affected it can result in mi, stroke, or limb loss or claudication from chronically limited circulation. Close f/u w doc.

Stop smoking, eat a Mediterranean diet, start exercising (walking, water aerobics, Tai Chi, yoga chair exercise as able). I would evaluate & treat & follow carotid, coronary, visceral, renal and extremity arteries to ensure no serious issues and control risk factors (BP. glucose, lipids, homocysteine, CRP).
