Most lisfranc injuries require surgery, as this can be an unstable midfoot injury which can lead to chronic midfoot pain and arthritis.
If the fracture is in normal alignment, you may be able to treat this with a cast, but if there is any degree of displacement (which there usually is) then fixation is preferable to help prevent long term midfoot problems.
