In brief: Heart rate

In pots there may a drop in BP upon standing but the bigger issue is a fast hr. In neurally mediated syncope (orthostasis ) there is a drop in BP that is associated with either no change in hr (vasodepresor), or a cardioinhibitory response where hr slws.
