Will they drug test a teenager before a knee arthroscopy? Will two days be sufficient to test negative?

In brief: Not necessarily

The hospital wont necessarily test a teenager for illicit substances.
But you need to be honest with the nurse and doctor as to a history of drug use. Certain substances are tested for particularly if there is an index of suspicion. Outside of that it depends on the drug as to how long it remains in the system.
