How are people who need a heart transplant selected?

In brief: Very carefully!

Seriously, they must have otherwise unreparable heart disease, heart failure, no other life limiting diseases:- infections , cancer, and willingness and ability to comply with a very strict medical regimen.

In brief: With Great Care

Transplant candidates are chosen based on: severity of heart failure, available treatment options remaining, their "support network" of family and friends, their overall physical health aside from the heart, avoidance of bad habits (smoking, drugs), lack of cancer, and unfortunately, insurance - a transplant costs ~$300, 000 just to get the heart, then $100k/yr in drugs.
