If this is a work related injury, you have to go through your employer or the nurse treatment coordinator that is assigned to your case.
If you work for a small company that has not such person, have your treating doctor try to get it approved throught the insurance company before they schedule the appointment. If you don't get approval from the work comp insurance company, they'll deny payment.
