Back pain can also *come from* other parts of your body.
This is called "referred pain." for example, a gall stone can cause pain near your right shoulder blade, gerd can cause pain between your shoulder blades and a kidney infection can cause low back pain.

Back pain does not necessarily "spread" to other areas, but certain causes of back pain (specifically nerve induced pain) can radiate from the back to other areas following a specific nerve pattern.
This is known as radicular pain. Best example is a "pinched nerve" in the back causing back pain and radiating leg pain, also known commonly as "sciatica." see your doctor if having back pain.
