My saline implant 5 years after mastectomy is flat and smaller. Could it have popped or leaked or could this be a medical issue? What should i do?

Breast implants do rupture. If it is a saline implant, there is no consequence to your health. Ruptured implant can be replaced.

A leaking implant would not be refilled, but it should be replaced.
