Adipose tissue (fat) causes an increase in the circulation estrogen levels by converting androgen precursors into estrone.
The increased estrogen level is detected by the pituitary, causing ovulation to be inhibited. That inhibition can result in an increase in the number of small cysts on the ovary - hence polycystic ovarian syndrome.
