In brief: Molar pregnancy

Once a molar pregnancy is thought likely by a combination of ultrasound and bhcg, a auction D&C should be done immediately.
Most molar pregnancies are discovered before 10 weeks gestation.

In brief: Depends.

In a molar pregnancy the placenta doesn't form properly and instead forms a tumor. It can be benign (lydatidiform mole) or malignant (choriocarcinoma).
Usually there is no fetus and the problem is detected in the first or second trimester. There can be a partial molar pregnancy where there is a placenta and fetus, which can rarely go to term.
