A truly non-functioning thyroid with almost no thyroid hormone synthesis will show many symptoms: most commonly dry hair/nails/skin, mental slowing, loss of eyebrows, weight gain, heart slowing, constipation, menstrual/sexual dysfunction, swelling, et al.
This definitely requires thyroid replacement. However, the most commonly seen mild hypothyroidism (tsh 4-10) usually has very few effects seen.
