In brief: No

Clexane does not clear existing blood clots, the body does.
Clexane prevents new clots from forming. Treatment of blood clots, once they have formed, should be no less than three months. Clexane is often used as an overlap with another blood thinner coumadin (warfarin). Once the Coumadin (warfarin) is working clexane can be stopped.

In brief: No

5 days of clexane? 150 mg? Who gave you this? Doesn't seem to serve the purpose and dosage is absurd.
Generally given as a bridge till the warfarin is therapeutic in keeping the blood thin enough, measured by inr; or rarely for entire six months; but in twice daily shots at 1mg/kg.
