Fractures of the kneecap are uncommon as it takes quite a bit of force to break the kneecap - if you do break it then you will have severe pain, swelling, tenderness and feeling of crepitus (like rice krispies under the skin).

Hi, pain is the big one.
Pain in the anterior aspect of the knee. Look for swelling, redness, and warmth over the patella or knee cap. If the fracture is bad enough and displaced there can be fracture gap and dysfunction of the extensor mechanism. In other words, it may be hard to straighten (or extend) your knee. After your doc checks you out, x-ray is the place to start the imaging evaluation.
