In brief: Severe sprain
Most, not all avulsion fractures around the ankle are treated in a manner very similar to a severe ankle sprain.
The ligaments that hold the bones of the ankle together can tear in the middle of the ligament, or they can pull off a small piece of bone where the ligament typically attaches to that bone. That is the most common cause of and avulsion fracture.

In brief: Serious injury.
The ligaments around the injury area were stronger than the bone they were attached to.
The ligaments held, but pulled a portion of the bone off.
