Nitrofurantoin or Macrodantin is excellent for sterilizing urine, however the medication can not acheive any appreciable tissue levels. As a result a patient can deelop pyelonephritis (kidney infection) if there are already any bacteria in the kidney tissues which will remain unaffected by nitrofurantoin.
