I have a tightness in the back of my throat and chest and pain when swallowing?

Looks like a problem. A primary care doctor, and ENT specialist, and a G.I. Specialist can help figure out what is causing swallowing problems. Possible causes include nerve problems, tumors (including cancers), infections, damage from acid reflux, esophagus malfunctions or spasms, eosinophilic esophagitis, etc... Normal swallowing is an automatic, smooth, painless event; so anything other than that should be examined.
