In brief: Need diagnosis

You describe three separate and possibly unrelated symptoms.
I am not sure about the pain when breathing but chest pain and trouble swallowing may be associated with reflux of acid from the stomach, so-called gerd. Pain when breathing may be related to muscle tenderness. Obviously, chest pain could be more serious and needs a diagnosis by your primary care physician.
