In brief: Usually cancer

Symptoms come, stay, and escalate, but having episodic events does not rule cancer out, but makes many other issues, from acute infectious to chronic irritable bowell, to inflammmatory bowel diseases more prominent.

In brief: It can.

However, at your age your risk of colon cancer is quite low.
Unless you have a very strong family history of colon cancer, with 1st or 2nd degree relatives developing colon cancer at a very young age, it is most likely something else. It can be something simple like the flu or a virus that attacks the small or large bowels. If it persists past 24-48 hrs you should visit your doctor.
