Probably the most common cause today is a vegetarian or vegan diet,both of which lack B12.
Absorption also a crucial cause ,caused by lack of intrinsic factor in stomach(crucial for absorption),use of some acid lowering medication(B12 not released from ingested food)Even under ideal situations ,less then 10%of ingested B12 will be taken up by the body.
