In brief: Physical exam
The spleen typically lies in the left upper abdomen, and under normal circumstances, in most people, cannot be felt or palpated as it is too high up behind the left rib cage.
When it is enlarged, the doctor may feel it by having you take a deep breath in then out-causing the diaphragm to contract, pushing the spleen down and allowing the tip to be palpated. Serial exams helpful to monitor.
