In brief: Hospice care is

Usually planned by the hospice team. This includes doctors, nurses, social workers, and clergy.
It is designed for people who have an approximately 6 month survival and who have either exhausted more aggressive care possibilites or are too frail. It is not limited to cancer. It is different than palliative care. There time is not critical and treatment is ongoing. Their goal is symptom management.

In brief: End of life care.

Hospice is meant for those who seemingly at the end of life, although that end can still be many months away.
The emphasis of hospice care is not trying to save the life, but to give comfort to the individual as their death approaches. the decision to pursue hospice is considered by the patient, family members and doctors.
