In brief: Bad stuff

Your heart rhythm is normally a carefully coordinated event that has a natural progression electrically to create an effective forceful heart beat.
When you have 3rd degree block, it's completely discordant (ie nothing is working together) . When the different parts of the heart are doing their own thing without communicating properly, it's life threatening and requires a pacemaker

In brief: Third degree

Third degree av block is complete interruption of conduction from atria to ventricles.
The ventricular rhythm is usual junctional escape though it may be idioventricular with rates less than 40bpm.
