While lung cancer can present like that, there are a fewl other causes for chest lymphadenopathy and a pulmonary nodule, including infectious and inflammatory diseases.
Without a doubt, further evaluation is mandatory, likely with a pet scan and likely a diagnostic biopsy. Of course the medical history is important (smoking, history of other cancers etc).
