In brief: Deceleration

Any impact that causes rapid deceleration of your head (going fast to a quick stop) such as car accident, fall, trauma from a hit (think football) or a direct impact to the skull can lead to a traumatic brain injury.
The faster the impact or deceleration, the worse the head injury is. Occasionally even minor blows to the head can cause bleeding which can lead to severe injury.

Falls are high on the list. Certainly combat /assault related causes. Ignorance of safety guidelines -- wearing helmet when on bike,skateboard,rollerblades,motorcycle. etc. CDC or Neurology site or TBI site will give most recent statistics.

Most common causes in my practice involves repetitive head trauma in sports, and motor vehicle accidents, but is very frequent in military, and can be seen with falling, direct blows over head, gunshot wounds, etc.
Seems that incidence is far higher these days than previously suspected.
