In brief: Years more training

A geriatrician(md or do), has completed a 3 year residency in internal medicine or family medicine then has done either a 1, 2 or 3 year specialized fellowship in geriatrics(as a cardiologist or other specialist would do).
After this training, one takes a board certifying test to prove ones expertise in the field. There are some docs that "grandfathered in" in the past without the fellowship.
