Emphysema is a pathological diagnosis when there is prove of rupture of septums between alveoli , causing large pockets of alveoli together (bullas of air). Most if not all patients c emphysema have copd.

In brief: Not much

COPD is more of a physiologic term, while emphysema refers to end-stage changes of the lungs, more anatomically.
Both terms are often used interchangeably.
