In brief: Infection

This is most likely due to an ear infection of the middle ear, otitis media, or ear canal (otitis external).
You should be on dry ear precautions. You need a hearing test. You may need a ct scan. Consider seeing an otologist.

In brief: Infected tissue

Granulomas, wherever they occur, are infected tissue.
On the ear drum area, they are often seen coming from ear tubes. It should be treated with a topical antibiotic and steroid combination like ciprodex. A follow-up exam should yhe occur by an ent, especially if no ear tube exists for a diagnosis and further treatment or biopsey if not resolved.
