What all can renal cell carcinoma spread to? And where would it most likely spread to first?

Lymph nodes first. Most of the time renal cell cancer is not detected in early stages, 25%- 30% cases already has metastasis at the time of diagnosis. As tumor grows invades capsule and spread to lymph nodes, then spread to liver, bone , lungs and brain.
Anywhere. Renal carcinomas can recur locally after surgery depending on the size and extent of the original tumor. A common site is lung, but it can affect liver, bone and other organs. One of the unusual sites that I have seen was a mass on the thigh the size of a football.
