In brief: Most qualify

Anybody who is a low surgical risk.
There are more than one cause for parkinson symptoms however. If your parkinson symptoms respond to sinemet, (carbidopa and levodopa) even if not as well anymore, your probably a good candidate.

In brief: Symptom dependent

The surgical procedure for parkinsons disease is called deep brain stimulation.
This is done after the drug therapies, that are the mainstay treatment, have been tried. The surgery is not a first line treatment. Talk to your neurologist about wether this technique is right for you.
