In brief: Level of care

Honestly there is considerable variation from place to place about how this works.
However if you want to get a sense of how things "ought" to work you should search for the "level of care and utilization system" of the american association of community psychiatrists. It is a bit complicated but covers the factors that determine medically appropriate residential care.

When a person is no longer able to function independently without support of others, then a facility should be explored. There are many different levels depending upon the need. However, one month tends to be what insurance will pay for, even if the need is greater. Also, there are residential homes for extended care that offer more independence, but still offer observation and support.
