In brief: Transient
Delirium is characterized by transient changes in sensorium in an otherwise cognitively intact person.
It is usually the result of a medical issue which is of a toxic or metabolic origin (ie. Infection, medications, hypoxia). Dementia is a chronic, usually progressive cognitive decline which persists over time. Obviously, both can occur in the same person.
