In brief: Depends...

Throwing up with good weight gain is usually from overfeeding.
Throwing up and not gaining weight is a sign of one or more of a large number of possible problems.

In brief: Physiologic reflux

Many babies spit up for good reason.
They are on a liquid diet, they are lying down most of the day, and their neuromuscular tone is not mature. The combination of these three things makes it easy for the stomach contents to come back up. Despite the fact that it's messy and occasionally the infants appear temporarily uncomfortable, most infants gain weight well and the reflux is not concerning.

In brief: Gaining weight

If the baby is gaining weight this is probaly benign.
If the baby is not gaining weight this could be a formula or milk allergy or intolerance, pyloric stenosis or other causes.
