In brief: Secretions

Your cough or sneeze can launch the germ into the air and you saliva carries it to anywhere your contaminated fingers touch.
Frequent hand washing and covering your cough/sneeze help diminish the spread. Direct person to person exposure can also do the deed. You are considered contagious until you have been on antibiotics for at least 24 hrs.

In brief: See below

Most commonly it spreads to neck lymph nodes.
It can however spread to adjacent soft tissues in the neck and rarely into the blood stream.
