Non operative treatment recommended, and includes bowel regimen (fiber, stool softners, laxatices to goal of 2 soft bowel movements daily) local sauves (prep h, desitin, (zinc oxide diaper cream) anti fungals, other) and hygiene (shower, hair dryer area daily). Surgery only if above fails. Post op pain is expected, manageable.

However, even if good analgesia is obtained the patient would still be better served by having some sedation, or deep sedation, for the procedure.
