In brief: Need a new one
There are ones that are soft inside and hard on the outside, they would last longer in your situation.
But if pain is there, you might nee to see a specilaist that treat TMJ problems.

In brief: Still grinding
It sounds like you are still grinding.
You might benefit from a different design. There are over 100 different designs and only 2 that reduce clenching or grinding. The others have no affect or may increase grinding. There are also other conservative ways to treat grinding. Finding a dentist well versed in treating bruxism would be my suggestion.

In brief: Not helping
If you've worn through the plate, you're still grinding.
That means you're not relaxing and are not in the jaw position when your grind will stop. Consider finding a dentist that can make you a neuromuscular orthosis. Otherwise, the muscles and joints may continue to suffer damage.

In brief: Get a new one
Biting through means that you're grinding.
..You'll periodically need new guards made as a matter of course. If your grinding is that frequent/forceful, you may need them made more frequently than someone who only grinds occassionally.

In brief: Keep wearing it
Even though you may have ground a small hole in the bite plate, it is still protecting your teeth.
Keep wearing it and see your dentist.
