You should get seen by your primary doctor to find out the cause. Possible causes are: chronic high blood pressure, undiagnosed diabetes, chronic nsaid's (e.g. Ibuprofen use..), polycystic kidneys, lupus, bladder/kidney obstruction, artery blockage (stenosis) etc...
