Have it evaluated! This may be considered an open fracture. These need to be treated aggressively and monitored carefully. The doctor may need to repair the nail bed and place you on antibiotics to prevent infection of the bone.
Clean and immobilize. Depending on which toe, the biggest worry is open fracture, so the nail needs to be removed and the area under the nail needs to be inspected to ascertain if there are any lacerations down to bone. If so, the area needs to be cleaned and stitched w/ oral antibiotics prescribed. If no laceration then the toe needs to be immobilized via buddy splinting to the adjacent toe vs. Post -op shoe.
