Paranoia is a more pervasively suspicious attitude, and colors one's expectations of most people and situations. "gut" instinct or intuition usually has more fluidity than this, and is not automatically predisposed to viewing others as "negative" in some way. Paranoia may also be accompanied by other symptoms, such as hallucinations.
