In brief: Many issues
The davinci robot is quite expensive and are not available at all hospitals.
Requires a ugeon dedicated to learn the technique and develop experience.

Not all hysterectomies are appropriate for the robot. If there is potentially cancer and the uterus is too large to fit through the vagina, a traditional laparotomy is required. If there is an ovarian mass which may be cancerous, a tradtiional laparotomy is also the recommendation.

I do a lot of robotic surgeries, but tend to limit its use to when I really need it to finish a case laparoscopically. Many hysterectomies can been done minimally invasive without the expense and trouble of using the robot. A total laparoscopic hysterectomy is the same surgery whether or not the robot is used to do it. It is a tool for surgery, not surgery in an of itself.

In brief: Varies
Not all hospitals have that equipment.
Not all physicians are trained for that procedure. Most women would have a better recovery with robotic hysterectomy and it is becoming more common and more available.

In brief: Training
There is extensive training needed to do a robotic hysterectomy and not all doctors have done the training.
Some hospitals may not have purchased the equipment which is quite expensive.
