In any surgery, the term "plasty" means "to shape or mold". This is actually where the specialty of plastic surgery gets it's name, not that we use plastic materials in surgery (most plastic sugery procedures do not involve synthetic implants). So an otoplasty literally means to shape or mold the ears (oto). It is most often done to fix protruding or prominent ears, but other deformities of the ears can be treated as well.

Prominent ears protrude due to 2 main issues.
The top of the ear lacks an "antihelical fold" which is the curved cartilage paralleling the upper rim of the ear. The bottom of the ear can protrude, because the "bowl" of the ear" just before the ear canal, is overly large. Otoplasty: a crease incision behind the ear, "bending" the cartilage at the top, +/-"tacking" the bowl down & back.
